how do you add door knobs and hinges to a door style

Believe it has to be a block as part of 'style'. It's a contianer for the geometry. The 'insert point' for the block is the subjective center of all geometry in the container. The style controls then work from that piont to place the block in the Style elements

 

Used this method for door handles quite a number of times, the real value, imho over just pasting the geometry for the handles in place, is you can create a multi-view block so in 3D you have the geometry visible, in all the 2d views an appropriate line drawing for that view. Most of the time just the front-elevation.

 

All fairly well documented in the help files. If you just need it for 3D, I'd just clone the handles and hinges out, position them to the door/jamb and create a group, if you want to 'open/close' the door
