    - website in flash or html or both?
    - do you regularly browse the web on a handheld device (iphone/ipad/android/bberry etc)

    - hardware expenditure per head per year
    - software expenditure per head per year

    - which colour working space is used?
    - how often is/are your monitor/s calibrated/recalibrated?
    - if any, which hardware calibration solution is used (eye1, colormunki, spyder etc)

    - which versions of software are being used? - max (2009, 2010, 2011 etc) and PS (CS3, CS4, CS5, +extended etc) and others
    - what kind of backup/redundancy do you use? (harddrive backup, digital offsite, dvd offsite, cloud storage)
    - which (if any) cloud storage solutions have you tried/used/regularly use? (dropbox, sugarsync , box.net)
    - how would you rate each of those storage solutions?

sure i'll think of more later.

For instance if someones work load or output of work increases 20%-30% in one year do they see and equally large increase in salary?
I'll likely launch a new salary survey early next year. I've been waiting for things to even out a bit more so I don't have to do it again right away.
