SandmanNinja
For the 'drop cornice' (for lack of a better word), just do the cornice the normal way (select the edges of the upper wall, convert to shape, then loft or sweep the cornice shape to the line) and then lower the loft or sweep. Then put some lights in the space between the top of the drop cornice and the ceiling. As I look at it again, I guess you'd need to have two lofts/sweeps - one for the lower cornice and one for the upper (thin and wide) cornice. To get the detail into the lower cornice, I'd have to Boolean it in (somehow).

mattclinch
by 'indirect' ceiling lighting i assume you are referring to the bounced light being emitted from the recess in the cornice. As Tanvir pointed out, this is best achieved with a Physical Area light, stretched out to act as a 'strip' light, similar to a cathode tube. Make sure you have lots of GI bounces to really kick that light all around the scene, given that it will need 2 or 3 to get itself out of the cornice area and illuminate your room. kippu
use vray lights inside the cornices pointing upwards and then some other vray lights outside the cornices pointing downwards.. and play with the values
