3D Studio Max Design 2015 - QuadColor_Startup.ms Error

Among all the errors I am getting in 3DS Max ... this is one of them I cannot solve.

 

I get this error when switching my UI - my settings are NOT being saved between sessions and it is frustrating. So, I have to constantly load my custom UI all the time.

 

On my main tool bar, I have created a NEW custom toolbar that has commonly used scripts on it (ie. floor gen, object ID, etc).

 

When I switch between UIs, I get this error.

 

No idea why it comes up or even how to fix it. Looked in the folder it lists ...\ENU\scripts\startup ... and the file is grayed out (because it should be hidden).

 

What is this ... and how do I fix it?
